Chapter 7 - The Ghost in the Operating Room

"You weren't in the crash, Ethan," Julian said, stepping forward and catching me by the elbow as my knees finally buckled. He guided me heavily down into the chair, keeping me from collapsing onto the linoleum floor.
"Then... then whose heart is coming?" I stammered, hyperventilating as tears blinded my vision. "Who was in that car?"
Julian let out a long, shuddering sigh, his eyes glistening with unshed moisture.
"Your brother, Lucas."
The name hit me like a physical explosion.
Lucas. My younger brother—the brilliant software engineer living downtown whom I hadn't spoken to in six months because of a stupid argument over family property.
"Lucas?" I screamed, the sound tearing raggedly from my throat. "My brother? No! He lives across town! Why would he be on I-94 at midnight?"
"Because he was coming to see you," Sophie whispered, her voice a broken wail as she pulled the oxygen cannula away from her face, gasping for air. "He called me this afternoon, Ethan. He found out about my diagnosis from Sarah. He knew you were too stubborn and too proud to ask for help, so he spent the last two days investigating experimental treatments behind your back. He was driving to your office to force you to face the truth... when the semi-truck ran him off the road."
The universe collapsed in on itself.
Every selfish choice I had ever made, every late night at the office, every bitter argument, every moment of emotional cowardice—it had all converged into this exact, agonizing point in space and time. My stubbornness had driven my wife into a fatal illness. My absence had driven my brother onto that highway in the middle of the night.
And now, my brother’s heart was being rushed up the elevator to save the wife I had abandoned.
"Prep the OR!" A frantic voice echoed over the hallway intercom system. "Transplant team alpha, report to surgical suite three immediately! Donor organ arrival in five minutes!"
The door flew open. Dr. Robert, the chief transplant surgeon, rushed in followed by an entire team of nurses pushing a specialized mobile cardiac containment unit.
"Dr. Vance, we're ready!" Dr. Robert called out, stopping short when he saw the chaos in the room. "We need to move the patient right now. Every second counts!"
Julian nodded grimly, stepping to the side of the bed to unlock the wheels of Sophie’s hospital cot.
Sophie reached out with both hands, grabbing my shirt as the nurses swarmed around her bed. Her eyes, filled with an unbearable, eternal love, burned into mine with a finality that terrified me.
"Ethan," she whispered over the rush of footsteps and equipment rolling into the room. "Live. Do not let his sacrifice—or my survival—be in vain. Love yourself... so you can finally learn how to love the world."
"Sophie! No! Don't leave me!" I shouted, lunging forward as the nurses wheeled her rapidly out of the room, the IV pole and monitor cables trailing behind like a lifeline being violently unwound.

Julian grabbed my shoulder, holding me back as I tried to follow them into the blindingly bright hallway.
"Let her go, Ethan," Julian said firmly, his own voice cracking with grief. "She is going into the operating room to carry your brother's heart inside her chest. And you... you are going to sit in this chair and wait for the miracle you never deserved."
