Chapter 1 - The Surgeon’s SecretDr. Arthur Hale’s face drained of color as the uniform steady green blips of the heart monitor illuminated the dark, sterile walls of Trauma Room 4. His stethoscope slipped from his pocket, hitting the linoleum with a hollow, plastic clatter.

"He... he was code blue," Hale stammered, his eyes darting from the monitor to the rise and fall of Dominic Russo’s chest. "The arterial line was completely flat. I called time of death."

"You called it too fast, Doctor," Cara said, her voice shaking with a mixture of adrenaline and sheer terror. Her fingers were still red with Dominic’s blood, pinned against the stainless steel edge of the table. "It wasn't hypovolemic shock alone. He had a tension pneumothorax combined with pericardial tamponade. The pressure collapsed his vena cava. His heart wasn't empty; it was choked."

Dominic’s pale blue eyes locked onto Hale. Even battered, bruised, and freshly dragged back from the edge of the abyss, the mafia boss radiated an icy, calculated menace. He didn't speak—he couldn't yet, with the chest tube draining fluid and air into a suction canister—but his gaze promised a slow, agonizing execution for anyone who had signed his death certificate prematurely.

"We need to get him to the ICU right now," Cara urged, stepping between Dominic and the terrified surgeon. "If Vincent Costello and his men find out he’s alive before we stabilize that airway, they’ll turn this hospital into a war zone. And Lorenzo... Lorenzo will finish what these bullets started."

Hale swallowed hard, wiping sweat from his forehead with the back of his blood-soaked sleeve. "If Vincent comes back and realizes I declared his boss dead to cover up a medical error—or worse, collusion—he’ll feed me to the gators in the Salt River. We have to hide him."

Before Cara could reply, the heavy doors of the trauma bay banged open.

Beatrice Gable stood in the threshold, her face ghost-white, clutching a portable phone to her ear. "Dr. Hale! The front lobby is locked down, but the Phoenix police are swarming the perimeter. And... and you need to look at the security monitors outside the ambulance bay. Lorenzo Russo just pulled up with six more cars."

Dominic’s hand reached out, his long, scarred fingers gripping the hem of Cara’s scrub top with surprising strength. He pulled her down until his lips brushed her ear, his voice a low, gravelly rasp that sent a shiver straight down her spine.

"Listen to me, nurse," Dominic whispered, his breath hot against her skin. "If Lorenzo finds me breathing in this room before sunrise, Phoenix will burn to the ash. Do exactly what I tell you, or we all die tonight."
