Chapter 5 - The Price of Blood

The private medical wing of the Castelliano mansion smelled of ozone, bleach, and death.
Dominic stood outside the operating room doors, his hands, shirt, and face covered in Isabella’s blood. He refused to wash it off. It was a physical manifestation of his failure. He had promised to protect her, had locked her in a cage to keep her safe, and his own world had still managed to poison her.
Marco stepped into the hallway, his face grim. "We interrogated the waiter’s family before we disposed of them, Boss. He was paid by the Lucchese family. It wasn't meant to kill Isabella. It was an abortifacient. A heavy dose of a black-market toxin meant to terminate the pregnancy. Lucchese wanted to take your heir away before it could even breathe."
Dominic didn't look up. He was staring at his bloody hands. When he spoke, his voice didn't sound human. It sounded like something crawled out from the deepest, darkest pits of hell.
"Assemble the men," Dominic whispered.
"Boss?"
"Every soldier. Every captain. Every hitman on our payroll," Dominic said, slowly raising his head. His eyes were completely bloodshot, devoid of any mercy, light, or sanity. "I want the Lucchese family wiped off the map by sunrise. Not just the Don. His captains, his associates, his warehouses, his home. Burn it all. If my child dies tonight, New York will drown in blood."
"Understood, Boss," Marco said, a chill running down his spine. He turned and walked away to unleash a war the city would never forget.
For three agonizing hours, Dominic stood by the glass window, watching the doctors work on Isabella. He watched them pump her stomach, inject fluids, and monitor the tiny, fragile heartbeat of the fetus on the monitor. The line on the monitor dipped dangerously low, flattening for a terrifying ten seconds before bouncing back up with a weak, irregular thump.
Thump... thump... thump...
Dominic pressed his forehead against the cold glass, tears finally slipping through his blood-streaked face. "Please," he whispered, a man who had never prayed in his life begging a silent universe. "Take me. Kill me. Let the Lucchese family take my territories. Just let them live. Let my family live."
At 4:00 AM, the lead surgeon, Dr. Evans, stepped out of the room, wiping the sweat from his brow.
Dominic grabbed the doctor by his scrubs, lifting him slightly off the floor. "Tell me."
"She’s stable, Mr. Castelliano," the doctor said quickly, fearing for his life. "The toxin was neutralized just in time. The baby... it’s a miracle, but the heartbeat is stabilizing. The child survived. But she lost a lot of blood. She’s in a medically induced coma to let her body heal."
Dominic let go of the doctor, his knees almost buckling with a wave of sheer, overwhelming relief. He stumbled into the room, falling into the chair beside her bed. He took her cold, pale hand in his, kissing her knuckles over and over again.
"You're safe," he sobbed quietly, pressing his face into her mattress. "I've got you. I'm so sorry, Bella. I'm so sorry."
While Isabella slept, the city burned.
Before the sun could rise over Manhattan, the Lucchese crime family ceased to exist. Their compound in Long Island was reduced to a smoking crater of ash and rubble. Don Lucchese was found in his study, executed with a single, precise bullet to the forehead—Dominic’s signature calling card. The power dynamic of New York had been permanently shattered. The Castellianos were no longer just one of the Five Families. They were the apex predators.
Two days later, Isabella’s eyes fluttered open.
The bright lights of the medical room blinded her for a moment. She immediately panicked, her hand flying to her stomach.
"It's okay. It's okay, Bella. They're safe," a soft, raspy voice said.
She turned her head. Dominic was sitting beside her. He looked terrible. He hadn't shaved, his clothes were wrinkled, and his eyes were sunken with exhaustion. But when he looked at her, there was a gentleness she had never seen in him before—a profound, shattering reverence.
"The baby... is okay?" she whispered, her throat dry.
"The baby is strong. Just like its mother," Dominic said, a tear slipping down his cheek as he carefully placed his hand over hers on her stomach. "The people who did this are gone. Permanently. No one will ever threaten you or our child again, Bella. I swear it on my life."

Isabella looked at him, seeing the raw, bleeding soul of the monster who loved her. She realized then that she could never truly escape him. Not because his walls were too high or his guards too numerous.
But because he had tied his entire dark, violent world to her heartbeat.
