Chapter 1 - The Surgeon’s Fatal Mistake

Headline: "He thought he was untouchable—until the city’s most dangerous man put a price on his hands."

The sterile air of Operating Room 4 was thick with tension. Adrian Vale’s blood pressure was cratering, the heart monitor emitting a frantic, high-pitched rhythm that signaled impending arrest.

Dr. Julian Cross worked with aggressive, impatient movements. He was a man accustomed to applause, not pressure, and the armed men standing guard just outside the double doors were making him sweat.

"More suction, Mara!" Cross barked, his voice sharp beneath his mask. "You're lagging. If I lose him because of your incompetence, I’ll ensure you never work in a hospital again."

Mara stayed silent, her gloved hands steady despite the adrenaline coursing through her veins. She knew the truth: Cross was rushing. He was trying to show off for the spectators in the gallery, but his haste was making him sloppy.

Suddenly, the arterial line ruptured. A bright spray of blood painted the surgical drape.

"Clamping!" Cross shouted, his forceps diving into the cavity. But he was blind. He clamped down hard, aiming for the hepatic artery—but Mara’s sharp eyes caught the error instantly. He hadn't clamped the artery. He had clamped the portal vein, a mistake that would starve Adrian’s liver of oxygen and kill him within minutes.

"Dr. Cross, that's the portal vein," Mara whispered urgently. "The hemorrhage is deeper, near the posterior lobe."

Cross’s eyes flared with ego-driven rage above his mask. "Are you questioning my technique, nurse? I am the Chief of Surgery. You are a glorified assistant. Hold the retractors and shut your mouth."

"But doctor, the pressure—"

"I said, shut up!" Cross hissed.

Mara stepped back, her heart pounding. She looked at the monitor. Adrian’s heart rate was dropping. $50\text{ bpm}$. $45\text{ bpm}$.

Through the glass window of the OR, the large man Adrian had spoken to—his right-hand man, Viktor—was watching them like a hawk. He didn't understand the medical jargon, but he understood the sudden red alarms flashing on the screens.

"He's going into cardiac arrest," Mara said, her voice dropping all subservience. "Step aside, Dr. Cross."

"What did you say to me?"

Mara didn't wait. She knew that if Adrian died, Cross would blame her, and her mother's care facility fees would go unpaid. She would be ruined, and a man would be dead.

She reached past Cross’s hand, ignored his sharp gasp of outrage, and manually released the clamp. With a swift, practiced motion she had practiced a thousand times in her dreams, she reached into the surgical field, applied direct finger pressure to the actual torn artery, and halted the bleeding instantly.

The monitor stabilized. The heart rate slowly climbed back to $65\text{ bpm}$.

Cross stood frozen, his hands hovering in the air. His eyes were wide with a mixture of terror and humiliation. A mere nurse had just corrected a fatal error in front of his entire surgical staff.

"Get out," Cross whispered, his voice trembling with a terrifying, quiet fury. "Get out of my OR, Mara. You are fired."

Mara slowly removed her hands, leaving a sterile pack in place that would hold until a competent resident could suture it. She looked at Cross, then at the unconscious, pale face of Adrian Vale.

As she backed away and walked out of the operating room, Viktor stepped into her path in the hallway. His massive frame blocked the light.

"Is he alive?" Viktor asked, his voice like grinding stones.

"For now," Mara said quietly, her neck still aching from where Cross had yanked her hair. "But you need to watch his recovery very closely."

Viktor looked at her, his eyes drifting to the red mark beginning to bruise on her neck, and then back to the closed doors of the OR. "The boss told me to find out what the doctor is hiding. And I think I want to hear what you know, too."
