Chapter 1 - THE TENSION IN THE TRAUMA ROOM

The air in the trauma room turned to ice. Dr. Vane’s grip on Elena’s wrist tightened for a fraction of a second before the sheer presence of the man in the doorway forced him to let go. This was Silvio Vescari, Luca’s younger brother and the ruthless enforcer of the family. If Luca died, Silvio would inherit an empire—or burn the city down looking for revenge.
"You're interfering with a medical declaration, Mr. Vescari," Vane said, his voice tightly controlled but betraying a tremor. "The patient is gone."
"She says he isn't," Silvio replied, his gaze locked on Elena. "And I tend to trust people who still have a conscience. Nurse Marlowe, was it? You have exactly two minutes before I let my men inside this room. If my brother is dead, we all leave together. If he isn't, you better make sure he breathes."
Elena didn't waste another second. Her hands shook, but her mind cleared as she grabbed a large-bore needle from the emergency tray. She knew exactly what Vane had missed—or deliberately ignored. It wasn't just a thoracic injury; it was a tension pneumothorax. Air was trapped in Luca’s chest cavity, building immense pressure that was crushing his heart and preventing it from beating.
"What are you doing? That's unauthorized procedure!" Vane hissed, stepping forward to block her.
"Step back, Doctor," Silvio warned, his hand moving subtly beneath his tailored jacket.
Elena positioned the needle over Luca's second intercostal space, right above the rib. She prayed her assessment was right. If she was wrong, she would plunge a needle directly into the heart of the city’s most dangerous mafia boss and seal her own fate.
She drove the needle down.
Hiss.
A sharp, violent rush of trapped air escaped the cylinder. Almost instantly, the flatline tone on the monitor broke.
Beep... Beep... Beep...
A weak, erratic rhythm fluttered onto the screen. Luca’s chest rose with a ragged, sudden gasp. His eyes didn't open, but the deep blue tint on his lips began to recede into a pale, bruised purple.
"He's back," Elena breathed, wiping sweat from her forehead.
Silvio let out a slow exhale, a dark smile playing on his lips. He looked at Dr. Vane, whose face had gone completely bloodless. "Funny how a probationary nurse found a heartbeat you missed, Doctor. Almost makes me wonder what else you're missing."
Vane swallowed hard, stammering about anomalies and post-mortem spasms, but Silvio was no longer listening. He turned to Elena. "He needs surgery to stabilize. But he isn't staying in this public wing. We own the penthouse floor of St. Jude’s. You’re coming with us."
"I can't just leave my shift," Elena countered, her heart hammering. "I'm a floor nurse."
"Not anymore," Silvio said smoothly. "As of right now, you are Luca Vescari’s private medical handler. And given what just happened, you're the only person in this hospital I trust to keep him alive."

As they wheeled Luca out under the heavy guard of the suited men, Elena caught sight of Dr. Vane retreating to his private office, furiously dialing a number on his personal phone. The realization hit her like a physical blow: the declaration of death wasn't a mistake.
It was an execution. And by saving Luca, she had just put a massive target on her own back.
