Chapter 6 - THE TWISTED ENDGAME

Three days later, the dust had seemingly settled. St. Jude’s Hospital had covered up the incident as a "terrorist intrusion," keeping the Vescari family name completely out of the press. Luca Vescari was recovering rapidly in a private, undisclosed mansion outside the city, and Elena had been brought along to ensure his recovery, guarded day and night by Silvio’s most trusted men.
Marcus had cracked under interrogation. He gave up the names of the Maroni associates and the corrupt police captains on their payroll. The threat seemed neutralized.
Elena stood on the terrace of the estate, breathing in the fresh air, feeling a sense of relief for the first time in a week. Silvio walked out to join her, handing her an envelope filled with enough cash to ensure she never had to work another night shift in her life.
"You're free to go, Elena," Silvio said softly. "The threat is gone. The mole has been dealt with. My brother owes you his life, and the Vescari family always pays its debts."
"Thank you, Silvio," Elena said, taking the envelope. "I'm just glad it's over."
"Is it?"
A cold, familiar voice echoed from the doorway of the terrace. Elena turned around to see Luca Vescari sitting in a wheelchair, pushed by a new attendant. He looked frail, but his eyes were sharp, calculating, and predatory.
"There's one detail that doesn't make sense, Nurse Marlowe," Luca said, his voice quiet but carrying an immense weight. "Marcus confessed to everything. The ambushes, the bribes, the Maroni alliance. But he swore on his life that he never hired Dr. Vane. He didn't even know Vane was on duty that night."
Elena felt the blood drain from her face. The air grew suddenly still.
"Dr. Vane told me they had his daughter," Elena stammered, looking between Luca and Silvio. "He said if Luca woke up, the people he named would destroy everyone."
"Exactly," Luca smiled coldly. "Marcus didn't have that kind of leverage over a high-profile trauma surgeon. But do you know who did? Someone who managed the hospital's private offshore trust fund. Someone who knew exactly which night shift Marcus was planning the ambush, ensuring a specific, compromised doctor would be in the trauma room to declare me dead."
Luca looked directly at his younger brother.
Silvio didn't move. His expression transformed from the supportive, protective brother into a mask of pure, unadulterated malice. A slow, dark chuckle escaped his lips.
"You always were too smart for your own good, big brother," Silvio said, his voice losing all its warmth. He didn't reach for his gun—he knew he was surrounded by Luca’s loyalists now. "You were going to hand the empire over to a board of legitimate directors. You were going to ruin everything we built. I couldn't let you do that."
Elena took a step back, horrified. Silvio was the mastermind. He had orchestrated the entire event, using the Maroni family and Marcus as scapegoats. He had intended for Luca to die on that table, but when Elena unexpectedly revived him, Silvio had to pivot, playing the role of the loyal brother until he could find another way to finish the job.
"You played a dangerous game, Silvio," Luca said calmly. Two heavy-set guards stepped out from the shadows, blocking Silvio's exit. "But you forgot the first rule of this family. You never strike the king unless you're certain he won't get back up."
Silvio looked at Elena one last time, a bitter, mocking smirk on his face. "You really should have left him dead, Nurse. You have no idea what you've just unleashed on this city."
As the guards dragged Silvio away into the basement of the estate, Luca turned his gaze to Elena.
"You did your job beautifully, Elena," the Mafia boss said, adjusting the blanket over his knees. "But now you know too much about the internal affairs of the Vescari family. I'm afraid your retirement from nursing is going to be... permanent. Welcome to the family business."

Elena looked down at the envelope of money in her hands, realizing it wasn't a reward for her freedom.
It was her first retainer as the underworld's exclusive medical shadow. She had saved the monster, and now, she belonged to him.
