Chapter 2 - THE VIP PENTHOUSE PRISON

The VIP wing of the hospital felt less like a medical sanctuary and more like a high-security fortress. Armed guards stood at every corner, their jackets bulging with concealed weapons. Luca Vescari lay hooked up to a private ventilator, surrounded by state-of-the-art monitoring equipment that money could buy, but human greed could still corrupt.

Elena monitored the vitals every fifteen minutes. She hadn't slept in nearly twenty-four hours. Every time she closed her eyes, she heard the hiss of the air escaping Luca’s chest and saw the terrified, guilty look on Dr. Vane’s face.

Around 4:00 a.m., Silvio entered the room, carrying two paper cups of terrible hospital coffee. He handed one to her.

"You look like you're waiting for the floor to open up and swallow you," Silvio observed, leaning against the IV pole.

"I'm waiting for whoever paid Dr. Vane to realize I ruined their plan," Elena said bluntly, taking a sip of the bitter liquid.

Silvio’s eyes narrowed. "So you figured that out, did you?"

"I'm new, Mr. Vescari, not stupid. A tension pneumothorax is something a first-year medical student can spot. Dr. Vane has twenty years of trauma experience. He didn't miss it. He let it happen. He wanted your brother to stay dead on that table."

Silvio leaned in closer, his voice dropping to a dangerous whisper. "The Vescari family is facing a transition. Luca was setting up a meeting to expose a mole within our high-ranking council—someone who has been selling our shipping routes to the rival Maroni syndicate. Whoever ordered the hit tonight didn't expect Luca to survive the ambush. And they certainly didn't expect a rookie nurse to pull him back from the grave."

Before Elena could respond, the heart rate monitor beside the bed began to spike violently. Luca’s blood pressure plummeted.

Beep-beep-beep-beep!

Elena rushed to the bedside. "He's going into anaphylactic shock. His airway is closing!"

"How? He's on a strict saline drip and targeted antibiotics," Silvio demanded.

Elena ripped the IV line out of Luca’s arm, smelling the clear fluid dripping onto the linoleum. It didn't smell like standard antibiotic solution. It had a faint, sweet almond scent.

Potassium cyanide derivative. Infused directly into his secondary IV bag while she had been distracted talking to Silvio.

"Someone changed the bag," Elena cried, grabbing an EpiPen from her emergency kit and slamming it into Luca's thigh. "We have a traitor on this floor right now!"

Silvio drew a silenced pistol from his waistband, his expression turning predatory. "Lock the doors, Nurse. Nobody leaves this floor alive until I find out who touched that bag."
