Chapter 4 - THE BULLET AND THE STETHOSCOPE

The hallway outside the nursery turned into a killing floor.
Heavy boots stomped across the marble floor. Shouts in clipped tactical Spanish echoed through the vaulted ceilings.
"Sector B is compromised! Sweep the second floor! Leave no survivors!"
Nicolás pushed himself up from the floor, his movements fluid and deadly. He checked the magazine of his pistol, flicked off the safety, and looked back at Sofía, who was crouched behind the reinforced mahogany base, shielding baby Mateo with her own body.
"Stay here," Nicolás ordered, his voice dropping to a gravelly command. "Do not make a sound. If the door opens and it's not me, put a pillow over the baby's face so he doesn't cry, and crawl out the service vent behind the wardrobe."
"I'm not leaving you to die in a hallway, Mr. Velasco," Sofía said fiercely, ripping off the long cloth belt of her medical robe and tying Mateo securely against her chest so her hands were free. "And I'm certainly not going to sit here like a sitting duck."
Nicolás stared at her for a split second, a flicker of genuine shock crossing his features, before a dark, fierce grin broke across his face.
"God help me, you're insane," he muttered.
He kicked the nursery door open just as an operative in full tactical black gear raised an assault rifle in the corridor.
BANG.
Nicolás’s shot was instantaneous and precise. The operative dropped before he could pull the trigger. Nicolás snatched the fallen man’s MP5 submachine gun with his left hand, keeping his sidearm in his right, and motioned for Sofía to stay directly behind his pivot shoulder.
"Valeria, stay down by the hearth!" Nicolás shouted back into the room.
They moved down the darkened corridor, navigating the shadows of the Velasco mansion like ghosts. To anyone else, it was a maze of terror; to Sofía, whose mind mapped spatial logic under extreme pressure from years in chaotic public ERs, it was simply an anatomy of survival.
Suddenly, a shadow lunged from the doorway of the master suite.
An assassin swung the butt of a shotgun toward Nicolás’s head. Nicolás blocked with his forearm, but the impact staggered him. The operative raised his weapon for the kill shot—
THWACK.
A heavy, metallic weight smashed into the side of the assassin’s temple with a sickening crunch.
The man crumpled instantly.
Nicolás blinked, turning his head. Sofía stood over the unconscious intruder, holding her heavy, solid-brass vintage medical diagnostic kit by its reinforced leather strap like a medieval mace.
"You just cracked a mercenary's skull with a doctor's bag," Nicolás breathed, a low chuckle escaping his lips despite the hail of bullets flying down the east wing.
"In the emergency ward of Hospital General, you learn that heavy equipment makes a great equalizer," Sofía panted, not missing a beat as she checked the man’s pulse to make sure he was out cold. "Keep moving!"
They reached the grand staircase, but the ground floor was already swarming. At least half a dozen armed men in black tactical gear were securing the foyer, holding Don Esteban and the remaining household staff at gunpoint.
"Boss says burn the estate to the ground," one of the men shouted over the comms units. "Find the boy and the nurse. The old lady gave us the code, but the boss wants no loose ends!"
Leonor.
Even locked away in the east wing, the bitter matriarch had sold out her own blood to a rival syndicate just to reclaim her throne.
Nicolás’s eyes went completely dead. The last shred of familial mercy inside him snapped, turning his soul into pure, unadulterated ice.
"Hold my belt," Nicolás whispered to Sofía.
"What are you doing?"
"Taking my house back."
Before Sofía could protest, Nicolás stepped out onto the open balcony overlooking the grand foyer, raised the MP5 submachine gun with one hand, and opened fire with terrifying, calculated precision.
The element of surprise shattered the mercenary squad in seconds. Three men dropped before they could even aim their rifles upward. Chaos erupted below as the remaining operatives scrambled for cover behind marble pillars and antique sofas.

"Now, Sofía! Run for the service elevator behind the library!" Nicolás roared, covering her retreat with a blistering wall of lead.
Sofía clutched baby Mateo tighter to her chest, ducking low, and sprinted down the cross-corridor toward the library, her heart screaming in her ears, realizing with stark, terrifying clarity: she had walked into this mansion to save a sick baby, but she had just stepped right into the middle of a civil war.
