Chapter 3 - The Price of Loyalty

The deafening report of the silenced pistol echoed inside the cramped room like a thunderclap.

Mateo didn't drop. Instead, his body jerked violently to the left as the bullet tore through the fleshy meat of his upper shoulder, shattering the collarbone and burying itself deep into the drywall behind him. A spray of crimson mist painted the green tiles.

"Mateo!" Alessandro roared, lunging forward on instinct.

"Stay back, boss!" Mateo grunted through clenched teeth, sliding down the wall, leaving a smeared streak of dark blood in his wake. Despite the crippling wound, his left hand shot out, snatching the heavy metal crash-cart by its rail and tipping it violently forward.

Surgical trays, IV stands, plastic basins, and heavy oxygen tanks cascaded across the floor in a chaotic avalanche of steel and plastic, right into Vincenzo’s path.

Vincenzo cursed, stumbling backward as a heavy steel basin caught him square in the shin. He fired wildly twice into the floor, the bullets shattering tiles and sending ceramic shrapnel flying.

"You stupid piece of trash!" Vincenzo screamed, regaining his balance and leveling the gun back at Alessandro's chest.

But in that fraction of a second—that tiny, razor-thin window bought with Mateo's blood—Alessandro had crossed the distance.

He didn't aim for the gun. He aimed for Vincenzo’s throat.

With the explosive power of a freight train, Alessandro crashed into Vincenzo, driving him backward through the remaining frame of the shattered doorway. The impact shattered the wooden jambs and sent both men tumbling violently out into the brightly lit, deserted hallway of the maternity ward.

Thud. Crash.

Alessandro was on top before Vincenzo could recover his breath. His massive, calloused hands wrapped around Vincenzo's throat like a vice grip.

"You touch my wife," Alessandro growled, his face inches from Vincenzo's bulging, terrified eyes, "you touch my child... and you think I’m just going to let you walk away?"

"You... you can't..." Vincenzo choked out, clawing frantically at Alessandro’s forearms, his fingernails tearing skin and drawing blood. "The... the Commission... they know I'm here... if I don't check in... the hit squad... they'll burn this entire hospital to the ground..."

"Let them come," Alessandro whispered, his eyes completely dead, devoid of human emotion, shining with the terrifying clarity of an apex predator defending its den. "I buried an army once. I can do it again."

Beneath him, Vincenzo’s eyes began to roll back. His frantic clawing slowed, his fingers slipping off Alessandro’s forearms as oxygen starved his brain.

Suddenly, a piercing, high-pitched wail shattered the heavy silence of the hallway.

It wasn't a gunshot. It wasn't an alarm.

It was the unmistakable, healthy, lungs-full-of-air cry of a newborn baby echoing from Delivery Room Four.

Alessandro froze. His hands loosened around Vincenzo’s neck just enough for the traitor to gasp in a ragged lungful of air.

From the doorway of the delivery room, the young resident appeared, pale as a ghost, holding a small, squirming bundle wrapped tightly in a blue flannel blanket.

"Mr... Mr. Moretti," the resident stammered, tears streaming down his face. "It's a boy. He's... he's healthy. Mrs. Moretti is asking for you."

Alessandro slowly stood up, his heavy charcoal coat stained with Mateo's blood and Vincenzo's sweat. He looked down at Vincenzo, who was coughing and gasping on the floor like a dying fish, then turned his eyes toward the doorway.

The weight of the underworld, the decades of violence, the blood on his hands—all of it seemed to fade into a hollow background noise as the baby’s cries grew louder.

"Tie him up," Alessandro commanded without looking back, his voice echoing down the corridor.

Mateo limped out of the room, holding his shattered shoulder with his left hand, a grim, blood-stained smile playing on his lips as he pulled a heavy nylon zip-tie from his pocket and stepped over Vincenzo.

"Consider it done, boss," Mateo rasped.

Alessandro took a deep breath, wiped a streak of blood from his cheek, and walked back into the room.
