Chapter 1 - The Ghost from the Operating RoomThe silence in Dominic Vale’s private office grew heavy, thick with the scent of aged bourbon and old leather. Rachel Caruso stood frozen near the door, her fingers digging into the fabric of her jacket.


"Six years," she repeated, the words tasting like ash. "You’ve been looking for me for six years? Why?"
Dominic didn’t answer immediately. He walked slowly around the mahogany desk, his tall frame cutting a shadow across the Persian rug. When he stopped just a few feet away from her, the sheer intensity in his gray eyes made her breath catch.
"Do you remember the night of October 14th?" Dominic asked softly.
Rachel’s mind raced back through years of blood, fluorescent lights, and panic. "I worked at St. Jude's General back then. Every night was a blur of trauma cases."
"Not this one," Dominic said, his voice dropping an octave. "A private airstrip outside Rockford. A botched execution. A man left bleeding out in the freezing rain with three hollow-point bullets in his chest and abdomen. The surgeons at the county hospital refused to touch him—they said he was dead on arrival, not worth the paperwork."
Rachel’s eyes widened. The memory slammed into her like a physical blow.
"I was the resident on duty," she whispered. "They brought him in on the floor of a van. It wasn't a hospital."
"No," Dominic agreed, a fierce pride flashing across his sharp features. "It was an abandoned hangar. And you didn't have a trauma bay, Dr. Caruso. You had a pocketknife, a flashlight, a bottle of industrial disinfectant, and hands that didn't shake while you sewed his aorta back together in the dark. That man was my younger brother, Julian."
Rachel stepped back, her heel hitting the heavy wooden door. "Julian... he survived?"
"Because of you," Dominic said. "And the moment I woke up from my own unconsciousness and asked who saved him, the nurses told me a furious young resident had cursed out three senior attendings, packed her bag, and vanished into thin air before anyone could get her name. All I had was a description: a fierce young woman with chestnut hair, green eyes, and hands that worked miracles."

"I left because the hospital administration threatened to strip my license for using unauthorized supplies," Rachel said defensively, her heart hammering against her ribs. "And I stayed hidden because your family's enemies were clearing out everyone connected to that shooting."
"And for six years, I built an empire just to find the doctor who defied death for my blood," Dominic murmured, stepping closer until she could feel the heat radiating from him. "Now, Rachel... you walk right into my club, call me a donkey head in Sicilian, and drop into my lap again."
