Chapter 1 - THE SCALPEL AND THE PHANTOM

The sterile scent of iodine and copper hung thick in Operating Room 4, heavy enough to choke a lesser man. To Dr. Ethan Caldwell, it was usually the scent of order, a predictable battleground where flesh, steel, and science obeyed his absolute will.

Tonight, it smelled like a execution chamber.

"Doctor! Her pressure is 70 over 40 and dropping!" the anesthesiologist barked, his voice piercing the steady, frantic beep-beep-beep of the heart monitor. "Fetal heart rates are decelerating. Twin A is at 80, Twin B is barely registering! We are losing them!"

Ethan didn't move. For three agonizing seconds, the surgical light above the table illuminated a ghost.

Hannah.

Her face was drained of all color, her lips a faint, bruised blue. The woman who had once lit up his bleak, overly manicured world with her effortless, unvarnished laughter was now dying under his hands. Her golden-brown hair, once thick and smelling of cheap vanilla shampoo, was matted with sweat and rain. And her body—god, her body—was broken. Under the surgical drapes, the sharp ridge of her collarbones protruded like thin glass. Her hands, resting limp beside her torso, were rough, callused, and scarred from endless shifts of manual labor.

What happened to you? the silent scream tore through his throat, but his mouth remained locked behind his surgical mask.

"Ethan!" Dr. Sarah Lin, the attending neonatologist, slammed her gloved hand onto the instrument tray, rattling the silver clamps. "Snap out of it! She’s hemorrhaging! If you don't cut now, you’re going to be pronouncing three deaths in five minutes!"

The shock shattered. A cold, ruthless adrenaline swept through Ethan’s veins, suppressing the billionaire heir, the regretful ex-lover, the grieving man, and leaving only the cold-blooded surgeon.

"Scalpel," Ethan commanded. His voice was a rasp of iron, trembling slightly before locking into a icy baseline.

The scrub nurse slapped the blade into his palm.

"Ten blade," Ethan muttered, his eyes narrowing to a laser focus. "Suction ready. Hang two more units of O-negative. Push epinephrine. If her heart stops, do not stop compressions until I get these babies out."

He pressed the steel into the pale skin of her lower abdomen.

The incision was swift, precise, and brutal. Blood welled immediately—dark, venous, pool-like. The abruption was catastrophic; the placenta had almost entirely detached from the uterine wall, drowning the infants in the very life force meant to sustain them.

"Suction! I can't see!" Ethan ordered, his fingers working inside the warm, bloody cavity with terrifying speed. "Get that blood out of there!"

The suction tube hissed, slurping up crimson streams. Ethan reached deep, his gloved hands navigating the chaotic, tear-filled landscape of her womb. His fingers brushed a tiny, fragile limb.

"Twin A coming out," he announced, his heart hammering against his ribs like a trapped beast.

He gently clamped and guided the first infant—a boy, tiny, blue, and terrifyingly limp. He cut the cord in a single flick of his shears and handed the miniature body to the waiting neonatal team.

"No cry! He’s not breathing!" a nurse yelled.

"Intubate! Bag him!" Sarah shouted from the warming station across the room.

Ethan couldn't look back. "Twin B!" he shouted, his hands plunging back into the hemorrhage. Twin B was deeper, jammed against Hannah’s pelvis. The cord was wrapped tightly around her tiny neck.

"Fetal bradycardia! Twin B’s heart rate is under 50!"

"Hold her down! She’s moving!"

Hannah’s body suddenly violently convulsed on the table despite the anesthesia. Her eyes snapped open—clouded, bloodshot, and wild with primal, instinctual panic. She couldn't speak through the oxygen mask, but her gaze locked onto the tall surgeon towering over her.

Through the haze of pain, drugs, and dying consciousness, Hannah saw him.

The dark eyes. The harsh, aristocratically sculpted brow above the blue mask. The man who had worn custom Savile Row suits while promising her eternity, now wearing a blood-soaked gown while holding her open.

A raspy, wet gurgle escaped her throat. Her pale, scarred hand reached out from the drapes, trembling violently, and grabbed the sleeve of his gown. Her fingers dug into his forearm with uncanny, terrifying strength.

"Don't... let... them..." she wheezed through the plastic mask. Her tear-streaked eyes pleaded with a desperation that shattered Ethan’s soul into a thousand jagged pieces. "Save... my... babies..."

"Hannah, look at me!" Ethan roared, leaning down, his voice breaching all professional decorum. "Look at me! I’m taking them out! I won't let you die! Do you hear me? I won't let them take you!"

"Ethan..." she whispered, her fingers suddenly slipping.

The heart monitor let out a single, terrifying, continuous line.

BEEEEEEEEEEEEEEEEEEEEEEEEEEEEP.

"She’s in V-fib! She’s cardiac arrested!" the anesthesiologist screamed. "Zero pulse!"

"Get those babies out NOW!" Sarah yelled.

Ethan pulled Twin B—a tiny girl—from the abyss, cutting the cord with trembling hands and handing her off without looking.

"Paddles!" Ethan screamed, stepping back from the blood-drenched field as the crash cart crashed into the side of the table. "Charge to 200!"

"Ethan, you need to step back, let the ER team—"

"I SAID CHARGE THE PADDLES!" Ethan bellowed, grabbing the handles himself. He looked down at the lifeless face of the only woman he had ever truly loved, the woman he had destroyed, now dying under his watch.

"Clear!"

He slammed the metallic pads onto her bare, pale chest. Her body jolt violently off the foam mattress.

BEEEEEEEEEEEEEEEEEEEEEEEEEEEEP.

Nothing. Flatline.

"Charge to 360!" Ethan’s eyes were bloodshot, sweat running down his temples, mixing with the spattered blood on his face shield. "Clear!"

THUMP.

Her body bounced. The line remained flat.

"Ethan, she’s been down for too long, the blood loss is too high—"

"DON'T YOU DARE DIE ON ME, HANNAH!" Ethan roared, throwing the paddles aside and slamming his bare, gloved palms onto her sternum, starting manual chest compressions with violent, desperate force. One, two, three, four. "Come back! Damn you, Hannah, come back to me!"

Across the room, a tiny, frail, high-pitched cry shattered the heavy silence.

Waaah... Waaaaah!

Twin A had gasped his first breath. A second later, a weaker, fainter cry echoed from the second warming bed.

At that exact second, the heart monitor sputtered.

Beep... beep... beep...

"Sinus rhythm restored!" the anesthesiologist gasped, slumping against his console. "We have a pulse! Weak, but it's there!"

Ethan stopped. He stood over her, his chest heaving, his hands covered in her blood, looking down at the faint, fragile rise and fall of her chest.

He leaned close to her ear, his voice breaking into a ragged sob that no one in that room had ever heard from the icy billionaire surgeon.

"You're safe," he whispered, his trembling thumb wiping a streak of crimson off her cheek. "I’ve got you, Hannah. And I am never letting you go again."
