Chapter 5 - THE FIRST PATIENT IN THE SHADOWSThe transition from dumpster-diving fugitive to resident physician of the Conti Syndicate was as jarring as waking up from a nightmare only to find yourself in a different one.

Sophie was given a fully equipped, state-of-the-art medical suite hidden beneath the East Wing of the estate. It had surgical lighting, advanced diagnostic machines, and pharmaceutical cabinets stocked with everything from basic trauma supplies to rare antibiotics.
For two weeks, she lived in a strange bubble of luxury and isolation. She spent her mornings reading medical journals, her afternoons playing with Coco—who now followed her everywhere like a tiny tan shadow—and her evenings waiting for the other shoe to drop.
It dropped on a rainy Tuesday night at 2:00 AM.
The heavy security doors to the underground medical suite buzzed loudly. Two guards rushed in, carrying a massive man whose left side was soaked in dark, arterial blood. It was Mateo, one of Alessandro’s top lieutenants, a man Sophie had seen standing guard in the dining room the night she saved the puppy.
"He took a slug to the shoulder and upper ribcage," one of the guards shouted, his face pale with panic. "The bullet missed the lung by a millimeter, but he's bleeding out internally!"
Sophie didn't hesitate. Months of rust vanished under the pressure of an immediate trauma. Her hands moved with practiced, mechanical efficiency.
"Get him on the operating table! Cut his shirt off! I need four units of O-negative blood, immediate suction, and prep the tray for a thoracotomy!"
For the next three hours, the underground suite transformed into a theater of high-stakes survival. Sophie worked with a fierce, absolute focus, her scalpel moving with the precision of a master artist. She could feel the watchful gaze of Alessandro, who had slipped into the observation gallery above the operating room, standing silently in the shadows with his arms crossed over his chest, watching her every move.
When she finally tied off the last internal bleed and closed the incision with meticulous sutures, her forehead was slick with sweat and her surgical gown was stained with blood.
She pulled off her bloody gloves, tossed them into the biohazard bin, and looked up toward the observation window.
Alessandro was gone.
Fifteen minutes later, as Sophie was washing her hands in the scrub sink, the heavy door of the suite opened. Alessandro walked in, stepping softly across the clean linoleum. He didn't look at Mateo, who was resting peacefully under the influence of heavy sedatives in the recovery bay. Instead, he walked straight up to Sophie.
He reached out, his warm fingers gently catching her wrist before she could pull away. He turned her hand over, examining the slight tremor in her fingers—the physical toll of three hours of intense, life-or-death surgery.
"You saved him," Alessandro said, his voice unusually soft.
"It was a standard trauma procedure," Sophie said, her breath catching slightly at his proximity. "Anyone with proper training could have—"
"No," Alessandro interrupted, his dark eyes locking onto hers with an intensity that made the air feel thin. "Most doctors would have panicked. Most men in my position would have lost a lieutenant tonight. But you... you are extraordinary, Sophie Chen."

Before she could form a reply, the secure comms panel on the wall buzzed violently.
One of the perimeter guards’ voices crackled through the speaker, breathless and laced with absolute panic: "Boss! We have a security breach at the main gates. A coordinated team... they’re bypassing the perimeter grid. It’s Moretti’s men. They found us."
