Chapter 1 - The Incision

The cold steel of the scalpel met baby Leo’s pale, translucent skin, the tiny tip guided by muscle memory built from thousands of high-stakes operations. In the suffocating silence of the nursery, only the erratic, dying whine of the heart monitor broke the tension. Rocco Marchetti’s Beretta remained pressed against Arya’s temple, the metal freezing against her skin, while Vanessa shrieked curses from the doorway and Bruno cocked his weapon.
"If his heart stops before you open that airway, I swear to God I’ll put a bullet through your skull first," Rocco rasped, his voice vibrating with raw, unfiltered terror.
"Shut up and hold his head!" Arya snapped back, her tone carrying an absolute, razor-sharp authority that instantly shocked the Mafia boss into absolute compliance.
She didn't hesitate. With a micro-adjustment of her wrist, she made a precise, millimeter-deep vertical stab incision through the cricothyroid membrane, bypassing the crushed and swollen upper airway. Dark, stagnant blood welled instantly, but she swept it aside with a sterile cotton swab held in her left hand, exposing the tiny, collapsed windpipe.
"Give me the 14-gauge angiocatheter, now!" she ordered, holding out her palm toward the cluster of paralyzed paramedics.
Miller, pale and sweating profusely, scrambled on the floor, grabbing a needle catheter from a spilled tray and slapping it into her hand with shaking fingers. Arya inserted it directly into the trachea. A sharp, hissing sound of pressurized air escaping filled the room.
"Bag him!" Arya yelled.
Miller lunged forward, slamming his hand onto the bag-valve mask, pumping oxygen directly into the makeshift port. For three agonizing seconds, nothing happened. The baby remained gray, his lips tinged with violet. Rocco stopped breathing, his finger tightening on the trigger of his gun.
Then, color rushed back into Leo's cheeks. The heart monitor’s chaotic shriek abruptly smoothed into a steady, rhythmic beep. Beep. Beep. Beep.
Leo let out a loud, furious, beautiful wail.

Rocco dropped his gun. It clattered heavily against the nursery floor. He collapsed forward, his massive hands trembling as he hovered over his son, tears carving clean paths through the soot and sweat on his face.
Arya pulled back, her hands covered in blood. She stood up slowly, her knees locking, and looked straight into the barrel of Bruno’s still-raised weapon.
