Chapter 18 - The Final Scalpel

As the jury retired to deliberate, a sudden commotion erupted near the court balcony.

Zachary, who had been sitting in the front row supporting me, suddenly gasped, clutching his chest. His face turned dangerously pale, and he collapsed heavily onto the wooden floor.

"Zachary!" I screamed, vaulting over the witness barrier and dropping to my knees beside him.

His pulse was thready, his breathing shallow and rapid. I quickly placed my ear to his chest. "Acute pericardial tamponade... he's bleeding around his heart! He needs an emergency thoracotomy right now!"

"The ambulance is ten minutes away!" a bailiff yelled into his radio.

"He doesn't have ten minutes!" I shouted, my surgical instinct taking complete control. "He has three minutes before his heart stops!"

I looked around the room. I grabbed a sterile pocket scalpel from my handbag—a tool I always carried—and called out to the court clerk. "I need an IV tube, a bottle of iodine, and a large-gauge needle from the emergency kit on the wall! NOW!"

The entire courtroom watched in breathless, stunned silence as I knelt over the man who had protected me. With absolute, unflinching precision, I sanitized his chest, located the exact anatomical space between his fifth and sixth ribs, and inserted the needle directly into the pericardial sac.

Dark red blood instantly surged into the syringe, relieving the fluid pressure crushing his heart.

Zachary let out a deep, gasping breath, his dark eyes fluttering open. He looked up into my eyes, his fingers reaching up to weakly touch my cheek.

"I told you..." Zachary whispered, a faint smile touching his lips, "...you're the hero, Dr. Sam."
