Chapter 1 - The First Cut

The scalpel gleamed under the glaring halogen lights, a cold sliver of steel that hovered mere centimeters above Hannah’s pale skin. In the sterile silence of Operating Room 4, the only sounds were the rhythmic, frantic beep of the fetal monitors and the low, mechanical hiss of the anesthesia machine.
Ethan’s hand was rock steady, a trait that had earned him legendary status across Chicago's medical community, yet his chest heaved beneath his sterile gown. Five years. Five agonizing years of burying himself in textbooks, night shifts, and cold corporate galas had not dulled the razor-sharp ache of her memory. He still remembered the scent of rain on her hair, the desperate panic in her tear-filled eyes the night he threw her out, and the crushing emptiness of the apartment the next morning when he realized she had vanished without a trace.
Now, she was back—broken, bleeding, and fighting for her life on his table.
"Doctor Caldwell? We’re losing Twin A's heartbeat. Deceleration is severe," the resident warned, sweat beading on his forehead beneath his surgical cap.
Ethan snapped back to reality, casting his personal history into the abyss. Right now, he was a surgeon first, and an ex-lover second. "Clamp," he ordered, his voice dropping into the icy, authoritative register that commanded absolute obedience. "Move fast. We have less than three minutes before hypoxia sets in."
He made the incision with clinical precision. Blood welled instantly, signaling the brutal reality of the placental abruption. The placenta was tearing away from the uterine wall prematurely, cutting off oxygen and nutrients to the babies. As he cut through the layers of tissue, his mind raced with a terrifying, agonizing question that threatened to break his focus: Thirty-two weeks. Five years apart. Could those children... could they be his?
He pushed the thought aside, forcing every ounce of his adrenaline into his hands. With a deft flick of his wrist, he opened the uterus. The rush of amniotic fluid spilled over the surgical drapes.
"Suction!" Ethan barked.
He reached inside, his gloved fingers searching blindly in the warm, crimson tide. His fingers brushed against a tiny, kicking foot. A surge of raw, primal panic unlike anything he had ever experienced in the OR gripped his throat.
"Got the first one," Ethan whispered, pulling a tiny, silent infant from the womb. The baby was blue, completely limp, and alarmingly small.
"Neonatal team, take him!" Ethan shoved the tiny boy into the waiting hands of the NICU specialist, not even pausing to catch his breath. "He’s not breathing. Intubate now!"

Without a second to spare, he plunged his hands back into the surgical cavity, his heart hammering against his ribs like a trapped bird. "Second twin is wedged transversely," he muttered, sweat stinging his eyes. "I need to rotate him immediately or we lose them both."
Behind him, the monitors shrieked an uninterrupted, high-pitched alarm—the sound of flatlining hope.
